We do not accept workers compensation insurance.

Please call our office before your first appointment to verify that your particular insurance plan is accepted in our clinic(s).

New Patient Forms

New Patient forms must be filled out before your first appointment at Catawba Valley Healthcare. To download, you can just click on the New Patient Packet link here. If possible, print the packet out at home and send the completed forms as an email attachment to

If you are unable to print the New Patient Packet at home, please plan to arrive at least 30-40 minutes before your first appointment to fill out forms in our waiting room. 

In addition to the New Patient Packet, please bring the following to your first appointment:

  • Photo ID
  • Health insurance card
  • Social Security card
  • A list of your current medications
  • If you are without health insurance but are employed, bring proof of income:
    • Last paycheck stub
    • Last year’s tax filing

I Have an Appointment Scheduled with a Prescribing Provider

If you are a new patient coming in for a prescribing provider appointment, please arrive 30 minutes prior to your scheduled appointment time to ensure that our nursing staff has adequate time to take your vitals and collect your medical history information before your appointment.
